The Anne Carlsen Center provides individualized support and training in homes and communities across the region. Through our Community Based Services, ACC helps families stay together and empowers individuals to maintain and develop connections within home communities. We enlist the talents of a comprehensive team of professionals to meet the specific interests, needs and desired outcomes for each person we serve.
In-Home Support Services
The Center helps individuals with disabilities and their families build home lives of dignity, independence and well-being. Our staff provides services in the home through customized supports for individuals of all ages. We nurture abilities, teach life skills of independence, and
measure progress through a series of goals and objectives. The Center also teaches critical skills and routines to families and caregivers.
Client Skills

Day Supports
The Anne Carlsen Center is dedicated to helping adults with disabilities enjoy lives of quality at home and in their community. Our Day Support Services are designed to meet the interests, needs and desired outcomes for each person we serve. We empower individuals over the age of 21 by helping each person create a unique plan for independence and community involvement.

Behavioral Supports
The Anne Carlsen Center recognizes the challenges often associated with caring for an individual with disabilities, and offers behavior support services that help families and caregivers successfully navigate daily routines. Our highly trained and experienced staff works with individuals, families and caregivers to address a variety of behavioral issues.
Services
- Observations of the client across multiple times and settings
- Functional assessment of problem behaviors to determine why the problem occurs by identifying associated environmental factors
- Development of positive behavior support plans
- Training to help the client develop more functional, alternative skills
- Training to help caregivers effectively implement the intervention plan
- Collaboration with other professionals involved with the client (respite care, school staff, therapists, physicians)
Through these Community Based Services, the Center provides guidance, training and support throughout life’s journey—fostering achievement, dignity and limitless possibilities.
