In-Home and Day Supports

Anne Carlsen Center (ACC) is dedicated to helping individuals with disabilities, and their families, enjoy lives of quality at home and in their communities.  Through two established programs, In-Home Support for children and adults and adult Day Support, a unique plan is developed to meet the specific interests, needs and desired outcomes for each person we serve.  We empower individuals and families, from birth through their adult lives, by supporting each person’s plan for independence and community involvement.

Community-based supports helps individuals gain skills in the following areas:

  • Communicating more effectively.
  • Budgeting money and making purchases.
  • Participating in social and recreational activity groups after school.
  • Learning personal care, dressing and hygiene skills.
  • Learning safety skills at home and how to make those choices.
  • Learning social skills needed to be safe and part of their community, family, school and social settings.
  • Empowering children and adults to advocate for themselves.

We help develop skills and community connections by:

  • Supporting individuals to build and maintain meaningful relationships.
  • Training for use of public transportation.
  • Planning and carrying out shopping trips.
  • Banking and money management.
  • Instruction and practice keeping appointments.
  • The option of spiritual development.
  • Vocational skill building.
  • Self advocacy and safety skill development
  • Developing leisure and fitness interests.

The Anne Carlsen Center offers individualized care to children and adults through our Community Based Services, providing support and training in people’s homes.  Our highly trained staff provides:

  • Assessments and evaluations to develop appropriate supports that meet personal goals.
  • Program coordination.
  • Ongoing individualized skill training.
  • Community-based programming.
  • Daily living activities.
  • Leisure training and support.
  • Self-help skills.
  • Community access.
  • Community connection.
